Keep in mind in order to be a Scout in good standing with the Boy Scouts of America and for use at all campouts, activities, and summer camp, a "Revised" Annual BSA Health Form must be kept on file for each registered Scout. Also, completion of Authorization to Treat a Minor form allows for emergency medical services to treat your Scout if needed when he is on an outing.
